need help with making my own register page for a game i am hosting... i need help plz!?

i am trying to host a private server for a game i am playing and i need some help making a sign up page for this game.....i would this page to be online so people can sign up for my game.(also i need it so when some one signs up it goes directly into my data base, i am using a program called mysq) if u have any more questions let me know i hope some one can help me. thanks
